لوپرولید در طبقه خود داروی شماره ۱ تجویزشده در ایالات‌متحده * برای معالجه سرطان پیشرفته پروستات است. Leupromer® ۷.۵ میلی‌گرمی تستوسترونی را که باعث سرطان پروستات وابسته به تستوسترون می‌شود، سرکوب می‌کند. لوپرولید استات یک GnRHa است که به‌طور گسترده برای درمان بیماری‌های وابسته به هورمون در زنان و مردان استفاده می‌شود. تجویز طولانی‌مدت یک GnRHa منجر به تنظیم مجدد گیرنده‌های GnRH در غده هیپوفیز می‌شود که موجب سرکوب کامل هورمون لوتئین کننده (LH)، هورمون تحریک‌کننده فولیکول و استروئیدهای غده جنسی پس از یک مرحله تحریک اولیه (شعله‌ور شدن هورمون) می‌شود. معمولاً طی ۴ هفته پس از شروع درمان با لوپرولید استات، تستوسترون سرم به میزان تقریباً برابر با جراحی کاستراسیون کاهش می‌یابد.

آندومتریوز در خانم‌ها

در رشته زنان و زایمان لوپرولید به‌عنوان یکی از بهترین گزینه‌ها برای درمان آندومتریوز ترویج می‌شود. تزریق Leupromer® ۷.۵ میلی‌گرمی به مدت ۱ ماه در مدیریت آندومتریوز ازجمله تسکین درد و کاهش ضایعات آندومتر انجام می‌شود. Leupromer® ۷.۵ میلی‌گرمی به همراه روزانه ۵ میلی‌گرم نورورتیندرون استات (میزان باقی‌مانده *) همچنین برای مدیریت اولیه آندومتریوز و برای مدیریت عود کردن علائم پیشنهاد می‌شود.

آندومتریوز: در ۱۰٪ از خانم‌هایی که در سن فرزندآوری قرار دارند، نوعی اختلال زنان وجود دارد که ویژگی اصلی آن وجود بافتی مانند آندومتر در خارج از رحم یا حفره آندومتر می‌باشد. این ضایعات به‌طورمعمول در تخمدان‌ها مشاهده می‌شود اما طبق مطالعات در بسیاری از مناطق دیگر لگن هم رشد می‌کند. آندومتریوز در ۱۵-۲۵٪ موارد عامل درد لگن است و با علائم اصلی دیسمنوره ارتباط دارد. درمان اولیه توصیه‌شده حداکثر به میزان ۶ ماه است. تکرار درمان آندومتر هم باید به ۶ ماه محدود شود.

فیبروئید

تزریق Leupromer® ۷.۵ میلی‌گرمی به مدت ۱ ماه همراه با آهن درمانی قبل از عمل فیبروئید برای بهبود کم‌خونی به دلیل خونریزی واژن ناشی از فیبروئیدها استفاده می‌شود. اکثر پزشکان ممکن است به مدت یک ماه تنها استفاده از آهن را آزمایش کنند، زیرا برخی از بیماران مبتلابه کم‌خونی تنها با آهن بهبود می‌یابد. توصیه می‌شود. Leupromer® ۷.۵ میلی‌گرمی در بیماران مبتلابه فیبروید بیش از سه ماه مصرف نشود.

فیبرویدها، تومورهای خوش‌خیمی هستند که از سلول‌های ماهیچه صاف درون رحم ناشی می‌شوند. میوما شایع‌ترین نوع این‌گونه تومورهای خوش‌خیم است که در ۲۰-۵۰ درصد زنان در دوره باروری پیش می‌آید. همانند اندومتروز، علائم مشاهده‌شده شامل درد/فشار در لگن، قاعدگی دردناک، خونریزی و عدم کارکرد اندام‌های تولیدمثلی و مجاور است. میوما علت ۴۰ درصد از عمل‌های هیسترکتومی انجام‌شده در ایالات‌متحده است. همانند اندومتروز، این تومورها به محیط هورمونی خود حساس هستند و بنابراین از سرکوب هورمونی با آگونیست های GnRH، به‌خصوص لیوپرولید استات، جهت مدیریت این بیماری استفاده می‌شود. لیوپرولید استات عموماً به‌صورت یک اقدام پیش از عمل جراحی برای کوچک ساختن اندازه تومور انجام می‌شود.

گمان می‌رود که لیوپرولید استات سیگنالینگ استرادیول و پروژستروژن را در کاهش اندازه لیومیوم‌ رحمی را سرکوب می‌کند.

بلوغ زودرس مرکزی در کودکان (CPP)

تزریق Leupromer® ۷.۵ میلی‌گرمی به مدت ۱ ماهه برای معالجه کودکان دارای بلوغ زودرس مرکزی (CPP) تجویز می‌شود.

بلوغ زودرس را متوقف کنید.

دوران کودکی را ادامه دهید.

تزریقLeupromer® ۷.۵ میلی‌گرمی به مدت ۱ ماه باعث سرکوب مؤثر موردنیاز می‌شود.

مشکلات روان‌پزشکی در بیمارانی که آگونیست های GnRH از جمله Leupromer® ۷.۵ میلی‌گرمی را مصرف کرده‌اند، گزارش‌شده است. این مشکلات شامل ناتوانی عاطفی مانند گریه، تحریک‌پذیری، بی‌تاب بودن، عصبانیت و پرخاشگری است.

Leupromer® ۷.۵ mg suppresses the testosterone that causes testosterone-dependent prostate cancer cells to grow.

Leuprolide acetate is a GnRHa, which is widely used for the treatment of hormone-dependent conditions in men and women.

A chronic administration of a GnRHa leads to downregulation of GnRH receptors in the pituitary gland, which results in a complete suppression of luteinizing hormone (LH), follicle- stimulating hormone and gonadal steroids after an initial stimulatory phase (hormonal flare). Usually within 4 weeks of initiation of leuprolide acetate therapy, serum testosteroneis reduced to near surgically castrated levels.

WOMEN

Endometriosis

Leuprolide is promoted within the gynecological field as one of the best options for treating endometriosis.

Leupromer® ۷.۵ mg for 1-month administration is used the management of endometriosis, including pain relief and reducation of endometriotic lesions.

Leupromer® ۷.۵ mg with daily norethindrone acetate 5 mg (Add-back*) is also indicated for initial management of ndometriosis and for management of recurrence of symptoms.

Endometriosis: 10% of women who are of child-bearing age, is a gynecologic disorder defined by the presence of endometrial-like tissue located outside of the uterine or endometrial cavity. These lesions are typically found on the ovaries but have been known to grow in many other areas of the pelvic peritoneum Endometriosis accounts for 15-25% of the pelvic pain experienced by women and correlates with the major symptom of dysmenorrhea.

The recommended initial treatment is no more than 6 month. Repeat treatment for endometriosis should be limited to 6 months.

Fibroids

Leupromer® ۷.۵ mg for 1-month administration with iron therapy are used before Fibroid surgery to improve anemia due to vaginal bleeding from Fibroids. Most of doctors may consider a one-month trial of iron alone as some patients anemia will improve with iron alone. It is recommended that Leupromer® ۷.۵ mg not be used for more than 3 months in patients with Fibroids.

Fibroids, are benign tumors that arise from smooth muscle cells located within the uterus. Myomas are known to be the most common benign tumor and occur in 20-50% of women during their reproductive period. Much as with endometriosis, the symptoms observed include pelvic pressure/pain, dysmenorrhea, bleeding and dysfunction of the reproductive and adjacent organs. Myomas are responsible for 40% of hydsterectomies performed in the US. As with endometriosis, these tumars are responsive to their hormonal environment and therefore hormones sup-pression using GnRH agonists, especially leuprolide acetate, hs used in the management of this disease. Leuprolide acetate is typically used as a preoperative measure to shrink tumor size.

Leuprolide acetate is thought tosuppress estradiol andprogesterone signaling in reducing uterine leiomyoma size.

During the early phase of therapy, gonadotropins and sex steroids rise above baseline because of the initial symptoms of Puberty may be observed.

Psychiatric events have been reported in patients taking GnRH agonists, including Leupromer® ۷.۵ mg. Events include emotional lability, such as crying, irritability, impatience, anger, and aggression. Monitor for development or worsening of psychiatric symptoms during treatment.
